Hip Pain
Hip pain can be felt in the hip, groin, outer hip or buttock, and common causes are arthritis, bursitis, gluteal tendon problems, labral tears, impingement and muscle strains. Some hip pain actually comes from the low back. PAR Physical Therapy treats hip pain in Myrtle Beach and Murrells Inlet, South Carolina.

Hip pain when walking, and hip pain lying on your side
Where it hurts says a lot. Groin pain often points to the joint itself. Outer hip pain, especially hip pain lying on side at night, usually points to the tendons and bursa. Buttock pain may come from the back. Your therapist maps your pain, checks your hip motion and strength, and watches how you walk before choosing a plan.
What may be behind your hip pain
Hip arthritis
Wear in the joint causes stiffness and groin pain, and it is often worse after rest. Exercise helps strength and motion without wearing the joint out. See arthritis and joint pain.
Outer hip pain: bursitis and gluteal tendinopathy
Hip bursitis and gluteal tendinopathy, together called greater trochanteric pain syndrome, cause tenderness on the outside of the hip. Weak glute muscles and a lot of side-lying or crossed-leg sitting add to it.
Hip labral tear and hip impingement (FAI)
A labral tear is damage to the cartilage rim of the socket. Femoroacetabular impingement, or FAI, is when the ball and socket pinch. Both can cause catching, deep groin pain and pain with deep bending.
Groin pain and hip flexor strain
A hip flexor strain or groin strain follows sudden kicking, sprinting or lunging. See Sports Injury Rehab.
When hip pain starts in your back
Hip and spine share nerves, so the source is not always the hip.
Back pain
Stiff or irritated lower spine joints can refer pain to the hip and buttock. See Back Pain.
Sciatica
Pain that runs down the leg with tingling is covered on Sciatica.
Knee
The hip and knee overload each other. See Knee Pain.
Pelvic or tailbone pain
Pain deep in the pelvis or tailbone has a separate page: Pelvic Pain.
How we treat hip pain
We pair hands-on care with strengthening for the hip, glutes and core.
- Hands-on manual therapy and joint mobilization for the hip and low back
- Hip and glute strengthening, built up step by step
- Gait training so you walk with less strain
- Dry Needling for tight muscles around the hip, when it fits
- Golfers: a TPI review of hip rotation in the swing
Before and after hip replacement
Getting stronger before a hip replacement can make recovery easier, and after surgery therapy follows your surgeon's protocol. Details are on post-surgical rehab.
