Pelvic Pain

Pelvic pain is pain in the pelvis, genitals, tailbone or during sex, in women and in men, and it is often linked to pelvic floor muscles that are too tight rather than too weak. Physical therapy works alongside your gynecologist or urologist. Pelvic pain in women and in men

Names differ. The pelvic floor is often involved.

Women

Pain with a period, pain in the lower belly, pain with sex, or pain sitting. Endometriosis, vaginismus and nerve irritation can be part of it.

Men

Chronic pelvic pain syndrome and chronic prostatitis cause aching in the perineum, testicles or lower abdomen. Pelvic pain in men is often missed.

Either

Pudendal neuralgia is irritation of the pudendal nerve and causes burning pain, worse sitting.

Painful sex and how physical therapy helps

Painful sex, also called pain during intercourse or dyspareunia, is common and treatable. When pain makes the muscles clamp down, as in vaginismus, a cycle forms. Therapy breaks the cycle with relaxation, gentle hands-on work, breathing and graded practice at your pace. You stay in control, and you can stop at any moment.

Tailbone pain, endometriosis and prostatitis

Tailbone pain (coccyx pain)

Pain on sitting, often after a fall or childbirth. Therapy looks at the tailbone, pelvic floor and hips.

Endometriosis pain

Physical therapy is part of care alongside your doctor. It does not replace medical care for the condition, and it can ease the muscle pain around it.

Chronic prostatitis

Often better described as a pelvic pain problem with tight muscles. Your urologist rules out infection first.

How treatment works

The goal is to calm the area and reduce sensitivity.

  • Hands-on therapy, internal and external, only with your consent
  • Pelvic floor relaxation and down-training, with biofeedback
  • Trigger point release, and Dry Needling when a therapist advises it
  • Breathing and relaxation, and pain neuroscience education

How pelvic pain overlaps with back and hip pain

The pelvis connects the spine and the legs, so back pain, hip pain and pelvic pain often travel together. See Back Pain, Hip Pain and Chronic Pain when pain has lasted for months. Pain during pregnancy belongs on prenatal and postpartum care. The overview is on pelvic floor therapy.
