Pelvic Organ Prolapse

Pelvic organ prolapse happens when the bladder, uterus or rectum drops and presses into the vaginal wall. It feels like heaviness, pressure or a bulge, often worse at the end of the day or after lifting. Physical therapy can reduce symptoms, and you can call PAR Physical Therapy in Myrtle Beach and Murrells Inlet, South Carolina to ask whether our team treats prolapse.

The main types of prolapse

Bladder prolapse (cystocele)

A prolapsed bladder drops into the front wall of the vagina.

Uterine prolapse

The uterus descends into the vaginal canal.

Rectocele

The rectum bulges into the back wall of the vagina and can make bowel movements harder.

Symptoms, and when they are worse

Pelvic heaviness, a vaginal bulge, or the feeling of something falling out are the usual signs. They are often worse after standing all day, lifting or straining, and better after lying down. Some people also have leaking. If leaking is your main problem, see Incontinence.

What causes it

Several things stress the pelvic floor over the years.

Childbirth

Pregnancy and vaginal delivery stretch the support. See prenatal and postpartum care.

Menopause

Lower estrogen thins the supporting tissue.

Pressure

Heavy lifting, chronic constipation and chronic coughing push down on the floor.

What physical therapy can do

Therapy works with your gynecologist or urogynecologist, and with a pessary if you use one.

  • Prolapse exercises: pelvic floor muscle training, with biofeedback
  • Pressure management and breathing, so you do not bear down
  • Lifting mechanics for work, kids and groceries
  • Bowel habit training
  • A safe return to exercise, with the movements to modify at first

Before and after prolapse surgery

If surgery is planned, learning pelvic floor skills first can help. After surgery, therapy follows your surgeon's guidance on lifting and activity. General pelvic floor information is on pelvic floor therapy. Tell your doctor right away about severe pain or trouble emptying your bladder.
